Indications

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase
- Better acid control than racemic proton pump inhibitors
- Peak plasma levels occur at approximately 1.5 hours
- 97% bound to plasma proteins
- Extensively metabolized in the liver by CYP enzymes
- Plasma elimination half-life of approximately 1-1.5 hours
Dosage
- 20 mg or 40 mg Once Daily for healing of erosive esophagitis
- 20 mg Once Daily for maintenance of healing of erosive esophagitis
- 20 mg Once Daily for symptomatic GERD
- Triple Therapy: Esomeprazole 40 mg Once Daily, Amoxicillin 1000 mg Twice Daily, Clarithromycin 500 mg Twice Daily for Helicobacter pylori eradication
- Dose for Zollinger-Ellison Syndrome: 20-80 mg once daily
- 20-40 mg once daily for acid-related Dyspepsia
- 20 mg once daily for Duodenal ulcer, 20-40 mg once daily for Gastric ulcer
- IV Injection/Infusion dosage for various conditions
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension should be taken as per specific instructions
- IV Injection for specific conditions
Interaction
- Metabolized in the liver by CYP2C19 and CYP3A4
- Unlikely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4
- No clinically significant interactions with specific drugs
- Potential interference with CYP2C19
- Increased plasma levels observed with certain drug co-administrations
Contraindications
- Hypersensitivity to any of the formulation

- Symptomatic response does not preclude the presence of gastric malignancy
- Specific instructions for patients with difficulty swallowing capsules
- Use of antacids while taking esomeprazole
Use in Special Populations
- Safety and effectiveness not established in paediatric patients
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- No alteration in pharmacokinetics expected in patients with renal impairment
Overdose Effects
- Lethal dose observed in rats
- Signs of acute toxicity include reduced motor activity, respiratory changes, tremor, and convulsions
- No specific antidote
- Supportive and symptomatic treatment recommended
