Indications

• Treatment of endometriosis
• Reduction of pelvic pain
• Effects on endometrium and breast
• Reduction of growth effect of estrogen on endometrium

Pharmacology

• Agonist at progesterone receptor
• Progestagenic effect in endometrium
• Antiproliferative effects
• Immunologic effects
• Antiangiogenic effects
• Reduction of endogenous production of oestradiol
• Suppression of trophic effects of oestradiol
• Hyperprogestogenic endocrine environment
• Moderately hypoestrogenic endocrine environment
• Antagonist at androgen receptors

Dosage & Administration

• Dosage: 2 mg daily
• Continuous intake without any break
• Preferably taken at the same time each day
• Tablet-taking can be started on any day of the menstrual cycle
• Tablet must be taken continuously without regard to vaginal bleeding
• Replacement of missed tablet(s)
• Administration with liquid
• Dosage without interruption

Interaction

• Metabolized by cytochrome P450 3A4 system
• Effects of CYP3A4 inhibitors

Contraindications

• Hypersensitivity to dienogest
• Active venous thromboembolic disorder
• Arterial and cardiovascular disease
• Diabetes mellitus with vascular involvement
• Severe hepatic disease
• Liver tumors
• Sex hormone-dependent malignancies
• Undiagnosed vaginal bleeding

Side Effects

• Common undesirable effects
• Subsiding effects with duration of treatment
• Reported effects: headache, breast discomfort, depressed mood, acne, migraine, circulatory system disorder, palpitations, hypotension, nausea, abdominal pain, flatulence, weight increase, irritability, nervousness, altered mood

Pregnancy & Lactation

• Limited data from use in pregnant women
• No special risks on pregnancy and embryonic/fetal development
• Not recommended during lactation

Precautions & Warnings

• Exclusion of pregnancy before treatment
• Use of nonhormonal methods of contraception
• Changes in bleeding pattern
• Contraindication in patients with severe hepatic disease
• Inhibition of ovulation in the majority of patients

Use in Special Populations

• Not indicated in children prior to menarche
• Efficacy in adolescents not yet established
• No relevant indication for use in the geriatric population

Overdose Effects

• No risk of acute adverse effects in case of inadvertent intake of multiple daily therapeutic doses
• No specific antidote
• Well tolerated with higher doses in studies
