Indications
- Congestive heart failure after an acute myocardial infarction
- Hypertension
Pharmacology
- Selective binding to mineralocorticoid receptor
- Blocks binding of aldosterone
- Regulation of blood pressure and CV disease pathophysiology
- Effects on epithelial and nonepithelial tissues
Dosage & Administration
- Recommended starting dose of 50mg once daily
- Titration to target dose of 50mg once daily
- Administration with or without food
- Use alone or in combination with other antihypertensive agents
- Dosage increase in inadequate blood pressure response
Interaction
- May increase risk of hyperkalaemia with ACE inhibitors and/or angiotension receptor blocker
- May reduce antihypertensive effect with NSAIDs, glucocorticoids, tetracosactide
- Enhance hypotensive effect of alpha-1 blockers, TCAs, amifostine, baclofen, neuroleptics
- May increase plasma level with mild to moderate CYP3A4 inhibitors
Contraindications
- Hyperkalaemia
- Severe renal impairment
- Severe hepatic impairment
- Concomitant use with potent CYP3A4 inhibitors
- Potassium-sparing diuretics

- Stomach pain
- Nausea
- Cough
- Flu-like symptoms
- Symptoms of serious allergic reactions like rash, itching, swelling, severe dizziness, trouble breathing
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Eplerenone should be used during pregnancy only if potential benefit justifies potential risk to the fetus
- Concentration in human breast milk after oral administration is unknown
- Decision should be made whether to discontinue nursing or discontinue the drug
Precautions & Warnings
- Use with caution in hyperkalemia
- Severe kidney disease
- Diabetic patients with congestive heart failure after an acute myocardial infarction including those with proteinuria
Use in Special Populations
- Pediatric use: safety and effectiveness not established
- Geriatric use: no differences in overall incidence of effectivity or safety observed in elderly patients
Overdose Effects
- Symptoms: Hyperkalaemia, hypotension
- Management: Symptomatic and supportive treatment
- Admin activated charcoal
- Initiate standard therapy for hyperkalaemia
