Indications
- Treatment of anemia in Chronic Kidney Disease (CKD) patients on dialysis
- Treatment of anemia in Chronic Kidney Disease (CKD) patients not on dialysis
- Treatment of anemia in Zidovudine-treated HIV-infected patients
- Reduction of allogeneic RBC transfusions in patients undergoing elective noncardiac nonvascular surgery
- Prevention of anemia of prematurity in infants with specific birth weight and gestational age
Description
- Recombinant human erythropoietin (EPO) expressed in Chinese hamster ovary cells
- Has a 165 amino acid sequence identical to human urinary EPO
- Apparent molecular weight of erythropoietin is about 30,400 daltons
Dosage
- Evaluate iron status in all patients before and during treatment
- Maintain iron repletion
- Monitor hemoglobin levels at least weekly when initiating or adjusting therapy
- Consider hemoglobin rate of rise, rate of decline, and ESA responsiveness when adjusting therapy
- Do not increase the dose more frequently than once every 4 weeks
Administration
- Do not shake the product
- Protect syringe from light
- Inspect visually for particulate matter and discoloration prior to administration
Interaction
- Nipotin's effect may be potentiated by simultaneous therapeutic administration of a haematinic agent such as ferrous sulphate
Contraindications
- Uncontrolled hypertension
- Serious allergic reactions to Erythropoietin alfa
- Patients who develop Pure Red Cell Aplasia (PRCA) following treatment with any erythropoietin
Side Effects
- Adverse reactions in CKD patients: hypertension, arthralgia, muscle spasm, pyrexia, dizziness, medical device malfunction, vascular occlusion, upper respiratory tract infection
- Adverse reactions in Zidovudine-treated HIV-infected Patients: pyrexia, cough, rash, injection site irritation
- Adverse reactions in cancer patients on chemotherapy: nausea, vomiting, myalgia, arthralgia, stomatitis, cough, weight decrease, leukopenia, bone pain
- Adverse reactions in surgery patients: nausea, vomiting, pruritus, headache, injection site pain, chills, deep vein thrombosis, cough, hypertension
- Adverse reactions in premature infants: fall in serum ferritin values
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Use in pregnancy only if potential benefit outweighs potential risk to the fetus
- Not recommended for pregnant or lactating surgical patients participating in autologous blood predonation program
- Caution advised in nursing women
Overdose Effects
- Nipotin overdosage can cause hemoglobin levels above the desired level
- Manage with discontinuation or reduction of Nipotin dosage and/or with phlebotomy
- Cases of severe hypertension observed following overdose
