Pharmacology

  • Mechanism of Action: Proton pump inhibitor that suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase
  • Absorption: Occurrence of peak plasma levels at approximately 1.5 hours after oral administration
  • Distribution: 97% bound to plasma proteins
  • Metabolism: Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
  • Excretion: Plasma elimination half-life of approximately 1–1.5 hours

Dosage

  • Oral Dosage Form:
    • Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
    • Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
    • Symptomatic GERD: 20 mg Once Daily for 4 Weeks
    • Helicobacter Pylori Eradication: Triple Therapy - Esomeprazole 40 mg Once Daily for 10 days
    • Zollinger-Ellison Syndrome: 20-80 mg once daily
    • Acid-related Dyspepsia: 20-40 mg once daily
    • Duodenal Ulcer: 20 mg once daily for 2-4 weeks
    • Gastric Ulcer: 20-40 mg once daily for 4-8 weeks
  • IV Injection or IV Infusion:
    • GERD with Erosive Esophagitis - Adult Patients: 20 mg or 40 mg once daily
    • Pediatric Patients: Body weight <55 kg: 10 mg Once daily, Body Weight ≥55 kg: 20 mg once daily
    • Risk reduction of rebleeding of Gastric or Duodenal Ulcers following therapeutic endoscopy in adults: 80 mg

Administration

  • Tablet or capsule: swallowed whole and taken one hour before a meal
  • Oral Suspension: contents mixed with water and swallowed within 30 minutes
  • IV Injection: administrated intravenously over a period of at least 3 minutes

Interaction

  • Metabolism Inhibition: Potential interference with CYP2C19, the major Esomeprazole metabolizing enzyme
  • Contraindications: Hypersensitivity to any of the formulation

Pregnancy & Lactation

No adequate and well-controlled studies in pregnant women. Breastfeeding should be discontinued if the use of esomeprazole is considered essential

Precautions & Warnings

  • Symptomatic response to therapy with ESO does not preclude the presence of gastric malignancy
  • ESO capsules should be taken at least one hour before meals

Use in Special Populations

  • Paediatric Use: Safety and effectiveness not established
  • Geriatric Use: No overall differences in safety and efficacy observed between elderly and younger individuals
  • Hepatic Insufficiency: No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • Renal Insufficiency: The pharmacokinetics of ESO in patients with renal impairment are not expected to be altered

Overdose Effects

  • Lethal Dose in Rats: Single oral dose of 510 mg/kg
