Indications

  • Treatment of anemia in Chronic Kidney Disease (CKD) patients
  • Treatment of anemia in Zidovudine-treated HIV-infected patients
  • Treatment of anemia caused by myelosuppressive chemotherapy
  • Reduction of allogeneic RBC transfusions in elective surgery patients
  • Prevention of anemia in premature infants

Description

  • Recombinant human erythropoietin (EPO)
  • Expressed in Chinese hamster ovary cells
  • Molecular weight of about 30,400 daltons

Dosage

  • Evaluation of iron stores before and during treatment
  • Monitoring hemoglobin levels at least weekly
  • Dose adjustment based on hemoglobin response
  • Different starting doses for CKD patients on dialysis and not on dialysis
  • Starting dose for Zidovudine-treated HIV-infected Patients
  • Starting dose for patients on cancer chemotherapy
  • Starting dose for prevention of anemia of prematurity

Administration

  • Do not shake or freeze
  • Visually inspect for particulate matter and discoloration
  • Protect syringe from light
  • Discard unused portions

Interaction

  • Potentiation by therapeutic administration of haematinic agent

Contraindications

  • Uncontrolled hypertension
  • Serious allergic reactions to Erythropoietin alfa
  • Development of Pure Red Cell Aplasia (PRCA) following erythropoietin treatment

Side Effects

  • Hypertension, arthralgia, muscle spasm, pyrexia, dizziness
  • Nausea, vomiting, myalgia, stomatitis, cough
  • Headache, depression, hypokalemia, thrombosis
  • Injection site irritation, bone pain, hyperglycemia, dysphagia

Pregnancy & Lactation

  • Inadequate studies in pregnant women
  • Reproductive toxicology in animals
  • Caution in nursing women
  • Consideration of breast-feeding benefits and therapy benefits

Overdose Effects

  • Hemoglobin level increase
  • Management with discontinuation or reduction of dosage
  • Management with phlebotomy if clinically indicated
  • Cases of severe hypertension
