Indications
- Osteoarthritis
- Rheumatoid arthritis
- Ankylosing spondylitis
- Acute gouty arthritis
- Moderate pain associated with dental surgery
Pharmacology
- Orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 decreases clinical signs and symptoms
- Decreased potential for GI toxicity
- No inhibition of COX-1 at doses up to 150 mg daily
Dosage & Administration
- Recommended doses for different conditions
- Patients may require additional postoperative analgesia
- Shortest duration and lowest effective daily dose should be used
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in the effect of diuretics and other antihypertensive drugs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential nephrotoxic effect with ciclosporin or tacrolimus
- Decreased lithium renal excretion leading to increased lithium plasma levels
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or GI bleeding
- Bronchospasm, allergic-type reactions after taking NSAIDs
- Pregnancy, lactation, severe hepatic dysfunction, estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age, inflammatory bowel disease
- Congestive heart failure, hypertension, ischaemic heart disease, peripheral arterial disease, cerebrovascular disease
Side Effects
- Palpitation, fatigue, influenza-like symptoms, ecchymosis
- Dry mouth, taste disturbance, mouth ulcer, appetite and weight change
- Atrial fibrillation, transient ischaemic attack, chest pain, flushing, cough, dyspnoea, epistaxis
- Anxiety, impaired mental acuity, paraesthesia, electrolyte disturbance, myalgia, arthralgia
- Confusion, hallucinations
Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Excreted in the milk of lactating rats
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution with patients at risk of developing GI complications with NSAIDs
- Special consideration for patients with significant risk factors for cardiovascular events
- Potential reduction in prostaglandin formation and renal blood flow
- Caution with patients with cardiac failure, left ventricular dysfunction, hypertension, pre-existing edema
- Monitoring of renal function for patients at risk
- Monitoring for signs of hepatic insufficiency and hypersensitivity reactions
- Exercising caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed with single or multiple doses
- Usual supportive measures for overdose
