Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
Potent orally active cyclooxygenase-2 (COX-2) specific inhibitor
Dosage & Administration
- 30 mg once daily for Osteoarthritis
- 90 mg once daily for Rheumatoid arthritis and Ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis
- 90 mg once daily for postoperative dental surgery pain
Interaction
- Increased prothrombin time with oral anticoagulants
- Reduced effect of diuretics and antihypertensive drugs with NSAIDs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Possible increase in nephrotoxic effect of ciclosporin or tacrolimus with coadministration
- Decreased lithium renal excretion leading to increased plasma levels of lithium
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Hypersensitivity reactions to acetylsalicylic acid or NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease
- Congestive heart failure
- Uncontrolled hypertension
- Established cardiovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Not known whether excreted in human milk
- Excreted in the milk of lactating rats
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution in patients at risk of gastrointestinal complications with NSAIDs
- Careful consideration in patients with significant cardiovascular risk factors
- Potential impairment of renal function and renal blood flow
- Caution in patients with cardiac failure, left ventricular dysfunction, hypertension, or edema
- Monitoring of liver function
- Discontinuation at signs of hypersensitivity
- May mask fever and other signs of inflammation
- Caution when co-administering with warfarin
Overdose Effects
No significant toxicity observed up to 500 mg single dose and 150 mg/day for 21 days
