Indications

  • Symptomatic relief of Osteoarthritis (OA)
  • Symptomatic relief of Rheumatoid arthritis (RA)
  • Symptomatic relief of Ankylosing spondylitis
  • Symptomatic relief of acute gouty arthritis
  • Short-term treatment of moderate pain associated with dental surgery

Pharmacology

  • Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
  • Selective inhibition of COX-2 without inhibition of COX-1
  • Decreases clinical signs and symptoms with decreased potential for GI toxicity and effects on platelet aggregation
  • Dose-dependent inhibition of COX-2 without inhibition of COX-1 at doses up to 150 mg daily

Dosage & Administration

  • Recommended doses for Osteoarthritis, Rheumatoid arthritis, Ankylosing spondylitis, Acute gouty arthritis, and postoperative dental surgery pain
  • Advisable to use the shortest duration possible and the lowest effective daily dose
  • Re-evaluate the patient's need for symptomatic relief and response to therapy periodically

Interaction

  • Increase in prothrombin time with subjects stabilized on chronic warfarin therapy
  • Potential reduction in the effect of diuretics and other antihypertensive drugs
  • Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
  • Possible increase in the nephrotoxic effect of ciclosporin or tacrolimus

Contraindications

  • Hypersensitivity to the active substance or excipients
  • Active peptic ulceration or active GI bleeding
  • History of allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors
  • Pregnancy, lactation, severe hepatic dysfunction, and estimated renal creatinine clearance <30 ml/min

  • Taste disturbance
  • Atrial fibrillation
  • Chest pain
  • Myalgia and arthralgia
  • Confusion and hallucinations (very rarely)

Pregnancy & Lactation

  • Not recommended in women attempting to conceive
  • Excreted in the milk of lactating rats
  • Women who use it must not breastfeed

Precautions & Warnings

  • Caution with patients at risk of developing gastrointestinal complications with NSAIDs
  • Careful consideration for patients with significant cardiovascular risk factors
  • Monitoring of renal function in patients with impaired renal function
  • Caution with patients having a history of cardiac failure, left ventricular dysfunction, or hypertension

Overdose Effects

  • Single doses of up to 500 mg and multiple doses of up to 150 mg/day did not result in significant toxicity
