Indications
- Osteoarthritis
- Rheumatoid arthritis
- Ankylosing spondylitis
- Acute gouty arthritis
- Moderate pain associated with dental surgery
Pharmacology
- Orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Inhibition of COX-2 without inhibition of COX-1
- Decreased potential for gastric damage and effects on platelet aggregation
- Dose-dependent inhibition of COX-2 without inhibition of COX-1
- Did not inhibit gastric prostaglandin synthesis
Dosage & Administration
- Recommended dose for osteoarthritis: 30 mg once daily
- Recommended dose for rheumatoid arthritis: 90 mg once daily
- Recommended dose for acute gouty arthritis: 120 mg once daily
- Recommended dose for dental surgery pain: 90 mg once daily
- Limited to a maximum of 3 days for postoperative dental surgery pain
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in the effect of diuretics and other antihypertensive drugs with NSAIDs
- Concomitant use with low-dose acetylsalicylic acid for cardiovascular prophylaxis
- Potential increase in nephrotoxic effect of ciclosporin or tacrolimus
- Decreased lithium renal excretion leading to increased lithium plasma levels
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or gastrointestinal bleeding
- Patients with certain allergic-type reactions after taking NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance less than 30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure
- Uncontrolled hypertension
- Established ischemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Precautions & Warnings
- Caution with patients at risk of gastrointestinal complication with NSAIDs
- Caution with patients at significant risk for cardiovascular events
- Reduced prostaglandin formation and impaired renal function
- Caution in patients with cardiac failure, ventricular dysfunction, or hypertension
- Monitoring of hepatic function and discontinuation upon hepatic insufficiency signs
- Discontinuation at the first appearance of hypersensitivity signs
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- Single doses of up to 500 mg did not result in significant toxicity
- Usual supportive measures recommended in the event of overdose
