Indications
- Symptomatic relief of Osteoarthritis (OA)
- Symptomatic relief of Rheumatoid arthritis (RA)
- Symptomatic relief of Ankylosing spondylitis
- Symptomatic relief of acute gouty arthritis
- Short-term treatment of moderate pain associated with dental surgery
Pharmacology
- Potent, orally active cyclooxygenase-2 (COX-2) specific inhibitor
- Decreases clinical signs and symptoms
- Decreased potential for GI toxicity and effects on platelet aggregation
- Primarily responsible for the synthesis of prostanoid mediators of pain, inflammation, and fever
Dosage & Administration
- Recommended doses for different conditions
- Possible need for increased dose in some patients
- Shortest duration and lowest effective daily dose should be used
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction of effect of diuretics and other antihypertensive drugs by NSAIDs
- Can be used concomitantly with low-dose acetylsalicylic acid for cardiovascular prophylaxis
Contraindications
- Hypersensitivity to the active substance or excipients
- Active peptic ulceration or active gastrointestinal (GI) bleeding
- Patients with certain allergic-type reactions after taking acetylsalicylic acid or NSAIDs including COX-2 inhibitors
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Inflammatory bowel disease
- Congestive heart failure (NYHA ll-IV)
- Established ischaemic heart disease, peripheral arterial disease, and/or cerebrovascular disease

Pregnancy & Lactation
- Not recommended in women attempting to conceive
- Possible excretion in human milk
- Excreted in the milk of lactating rats
- Not recommended for breastfeeding women
Precautions & Warnings
- Caution with the elderly and patients with prior history of gastrointestinal disease
- Treatment of patients with significant risk factors for cardiovascular events after careful consideration
- Possible reduction in prostaglandin formation and renal blood flow, impairing renal function
- Caution in patients with a history of cardiac failure, left ventricular dysfunction, or hypertension
- Monitoring of liver function and discontinuation upon signs of hepatic insufficiency
- Discontinuation at the first appearance of hypersensitivity signs
- May mask fever and other signs of inflammation
- Caution when co-administering with warfarin or other oral anticoagulants
Overdose Effects
- No significant toxicity observed with single and multiple doses
- Usual supportive measures recommended in case of overdose
