Indications
- Osteoarthritis
- Rheumatoid arthritis

Pharmacology
- Potent COX-2 specific inhibitor
- Decreases pain, inflammation, and fever
- Selective inhibition of COX-2
- Did not inhibit gastric prostaglandin synthesis
DosageAndAdministration
- Initial dose of 30 mg once daily for OA
- Increased dose of 60 mg once daily for insufficient relief
- 90 mg once daily for RA and ankylosing spondylitis
- 120 mg once daily for acute gouty arthritis
- 90 mg once daily for postoperative dental surgery pain, limited to a maximum of 3 days
Interaction
- Increase in prothrombin time with oral anticoagulants
- Reduction in the effect of diuretics and antihypertensive drugs
- Usage with low-dose acetylsalicylic acid is possible
- Potential increase in nephrotoxic effect of ciclosporin or tacrolimus
- Decrease in lithium renal excretion, leading to increased plasma levels
Contraindications
- Hypersensitivity to active substance or excipients
- Active peptic ulceration or GI bleeding
- History of allergic-type reactions to NSAIDs
- Pregnancy and lactation
- Severe hepatic dysfunction
- Estimated renal creatinine clearance <30 ml/min
- Children and adolescents under 16 years of age
- Inflammatory bowel disease
- Congestive heart failure, hypertension, established cardiovascular diseases
