Indications

  • Relief from chronic heartburn symptoms and other symptoms associated with GERD
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Zollinger-Ellison Syndrome
  • Acid-related Dyspepsia
  • Duodenal & Gastric ulcer

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Specific inhibition of the H+/K+-ATPase
  • Better acid control than racemic proton pump inhibitors
  • Enteric-coated pellet formulation
  • Peak plasma levels occur at approximately 1.5 hours
  • Metabolized in the liver by the cytochrome P450 enzyme system
  • 97% bound to plasma proteins
  • Systemic bioavailability is approximately 90%
  • Plasma elimination half-life is approximately 1-1.5 hours
  • Triple combination therapy with clarithromycin and amoxicillin

Dosage

  • Oral dosage form for healing of erosive esophagitis, symptomatic GERD, Helicobacter pylori eradication, Zollinger-Ellison Syndrome, acid-related dyspepsia, duodenal & gastric ulcer
  • IV Injection or IV infusion for GERD with erosive esophagitis and risk reduction of rebleeding of gastric or duodenal ulcers
  • Different dosages for adult and pediatric patients
  • Dosage adjustments for patients with hepatic insufficiency

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal
  • Oral suspension administration instructions
  • IV injection preparation and administration
  • Specific instructions for administration through a nasogastric or gastric tube

Interaction

  • Extensively metabolized in the liver by CYP2C19 and CYP3A4
  • Not likely to inhibit certain CYPs
  • Potential interference with CYP2C19
  • Inhibition of gastric acid secretion
  • Drug interactions with phenytoin, warfarin, quinidine, clarithromycin or amoxicillin
  • Potential interference with the absorption of certain drugs

  • No adequate and well-controlled studies in pregnant women

Precautions & Warnings

  • Symptomatic response to therapy does not preclude the presence of gastric malignancy
  • Specific instructions for patients taking esomeprazole capsules
  • Use of antacids
  • No established safety and effectiveness in pediatric patients
  • No overall differences in safety and efficacy observed between the elderly and younger individuals

Use in Special Populations
