Description
- A proton pump inhibitor used to relieve chronic heartburn symptoms and other symptoms associated with GERD, for the healing of erosive esophagitis, for maintenance of healing of erosive esophagitis, in combination with amoxicillin and clarithromycin for eradication of Helicobacter pylori infection, for Zollinger-Ellison Syndrome, acid-related Dyspepsia, duodenal and gastric ulcer

Pharmacology
- Proton pump inhibitor that suppresses gastric acid secretion
- Absorption: Peak plasma levels occur at approximately 1.5 hours after oral administration
- Distribution: 97% bound to plasma proteins
- Metabolism: Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
- Excretion: Plasma elimination half-life is approximately 1-1.5 hours
Dosage
- 20 mg or 40 mg oral dosage for healing of erosive esophagitis
- 20 mg oral dosage for maintenance of healing of erosive esophagitis
- 20 mg oral dosage for symptomatic GERD
- 40 mg oral dosage for eradication of Helicobacter pylori infection
- 20-80 mg oral dosage for Zollinger-Ellison Syndrome
- 20-40 mg oral dosage for acid-related Dyspepsia
- 20 mg oral dosage for duodenal ulcer
- Intravenous dosage for various conditions based on patient age and weight
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral Suspension: Contents mixed with water and consumed immediately
- IV Injection: Administered intravenously over a period of at least 3 minutes
Interaction
- Metabolized in the liver by CYP2C19 and CYP3A4
- No clinically relevant interactions with drugs metabolized by CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4 expected
- Potential interference with CYP2C19, leading to changes in clearance of certain drugs
- Co-administration with clarithromycin has resulted in increases in plasma levels of Epizol and 14-hydroxyclarithromycin
Contraindications
- Contraindicated in patients with known hypersensitivity to any of the formulation
Side effects
- Most frequently occurring adverse events include headache, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth
Pregnancy lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects observed in animal studies
- Excretion in milk not studied
- Breastfeeding should be discontinued if essential
Precautions warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Administration at least one hour before meals
- Use of antacids while taking Epizol
- Instructions for patients who have difficulty swallowing capsules
Special populations
- No established safety and effectiveness in pediatric patients
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for mild to moderate hepatic insufficiency
- No alterations expected in patients with renal impairment
Overdose effects
- Toxicity reported in animal studies at high doses
- Symptomatic and supportive treatment recommended in case of overdosage
- No specific antidote known

- Store at a temperature not exceeding 30°C in a dry place
