Indications
- Chronic management of hyperuricemia in patients with gout
- Not recommended for asymptomatic hyperuricemia
Pharmacology
- Non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
- Not expected to inhibit other enzymes involved in purine and pyrimidine synthesis and metabolism at therapeutic concentrations
Dosage & Administration
- Recommended starting dose of Febuxostat 40 mg tablet once daily
- Dose adjustment for different serum uric acid levels
- Management recommendations for tumor lysis syndrome and gout flares
- No dose adjustment needed for renal or hepatic impairment
- Safety and effectiveness not established in pediatric patients under 18 years of age
Interaction
- Contraindicated with mercaptopurine/azathioprine due to increased plasma concentrations
- No expected dose adjustment for co-administration with rosiglitazone/CYP2C8 substrates or naproxen
- Possible effect on metabolism with inducers/inhibitors of glucuronidation
- Co-administration recommendations for colchicine/indometacin/hydrochlorothiazide/warfarin and antacids
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Most commonly reported adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C with no adequate and well-controlled studies in pregnant women
- Caution advised for nursing women
Precautions & Warnings
- Increased gout flares during initiation of anti-hyperuricemic agents
- Monitor for signs and symptoms of MI and stroke due to higher rate of cardiovascular thromboembolic events
- Monitor liver function tests periodically
Overdose Effects
- Studied in healthy subjects up to 300 mg daily for seven days without evidence of dose-limiting toxicities
