Indications
- Relief of symptoms associated with seasonal and perennial allergic rhinitis
- Relief of symptoms associated with chronic idiopathic urticaria
Pharmacology
- Fexofenadine is an inverse agonist of the H1 receptor
- Potent and selective affinity for H1 receptors
Dosage & Administration
- Adults and children 12 years and older: 60 mg twice daily, 120 mg once daily, or 180 mg once daily (for allergic rhinitis)
- Children from 6 to 11 years: 30 mg twice daily or 60 mg once daily (for allergic rhinitis)
- Children from 2 to 11 years: 30 mg or 5 ml twice daily (suspension for allergic rhinitis)
- Adults and children 12 years and older: 60 mg twice daily, 120 mg once daily, or 180 mg once daily (for chronic idiopathic urticaria)
- Children from 6 to 11 years: 30 mg twice daily or 60 mg once daily (for chronic idiopathic urticaria)
- Children from 6 months to less than 2 years: 15 mg or 2.5 ml twice daily (suspension for chronic idiopathic urticaria)
- Children from 2 to 11 years: 30 mg or 5 ml twice daily (suspension for chronic idiopathic urticaria)
Interaction
- No hepatic interaction
- Coadministration with erythromycin or ketoconazole increases fexofenadine plasma level
- No interaction between fexofenadine and omeprazole
- Antacids containing aluminium and magnesium hydroxide reduce bioavailability
Contraindications
- Known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients
Side Effects
- Common: headache, drowsiness, dizziness, nausea, fatigue
- Hypersensitivity reactions with manifestations such as angioedema, chest tightness, dyspnoea, flushing and systemic anaphylaxis
- Insomnia, nervousness, tachycardia, palpitations, diarrhea, rash, urticaria, pruritus
Pregnancy & Lactation
- Limited data in pregnant women
- Fexofenadine was found to cross into human breast milk
- Not recommended for mothers breastfeeding their babies
Precautions & Warnings
- Careful administration in older people and renally or hepatically impaired patients
- Patients with a history of or ongoing cardiovascular disease should be warned
Use in Special Populations
- Pharmacokinetics of fexofenadine are altered in individuals with renal impairment
- A dose of 60 mg once daily is recommended as the starting dose in patients with decreased renal function
- Moderate to severe hepatic disease does not substantially affect the pharmacokinetics of fexofenadine
- Adverse events were similar in elderly patients
- Increased bioavailability in individuals over 65 years of age
Overdose Effects
- Reported effects include dizziness, drowsiness, fatigue, and dry mouth
- Maximum tolerated dose not established
- Symptomatic and supportive treatment recommended
