Indications
- Relief of symptoms associated with seasonal and perennial allergic rhinitis
- Relief of symptoms associated with chronic idiopathic urticaria
- Improves health-related quality of life and work/activity productivity
Pharmacology
- Fexofenadine is an inverse agonist of the H1 histamine receptor
- Has potent and selective affinity for H1 receptors
- Does not carry antidopaminergic, antiserotonergic, anticholinergic, sedative, or adrenergic blocking activity
- Unlikely to cause significant CNS effects
- Prevents activation of H1 receptors and downstream effects of histamine binding
Dosage & Administration
- Adults and children 12 years and older: 60 mg twice daily or 120 mg once daily or 180 mg once daily
- Children from 6 to 11 years: 30 mg twice daily or 60 mg once daily
- Children from 2 to 11 years: Suspension - 30 mg or 5 ml twice daily
- Dosage adjustments for impaired renal function
Interaction
- No hepatic biotransformation
- Coadministration with erythromycin or ketoconazole increases fexofenadine levels in plasma
- No interaction with omeprazole
- Administration of antacid containing aluminium and magnesium hydroxide 15 minutes prior causes reduction in bioavailability
Contraindications
- Known hypersensitivity to Fexofenadine Hydrochloride or any of its ingredients
Side Effects
- Nervous system disorders: headache, drowsiness, dizziness
- Gastrointestinal disorders: nausea
- Immune system disorders: hypersensitivity reactions
- Cardiac disorders: tachycardia, palpitations
- Skin and subcutaneous tissue disorders: rash, urticaria, pruritus
Pregnancy & Lactation
- Limited data in pregnant women
- No direct or indirect harmful effects in limited animal studies
- Not recommended for breastfeeding mothers
- No human data on effect on fertility available
Precautions & Warnings
- Limited data in older people and renally or hepatically impaired patients
- Administer with care in special groups
- Patients with history of or ongoing cardiovascular disease should be warned about potential adverse reactions
Use in Special Populations
- Pharmacokinetics altered in individuals with renal impairment
- Recommended starting dose in patients with decreased renal function: 60 mg once daily
- No substantial effect on pharmacokinetics in moderate to severe hepatic disease
- Similar adverse events in elderly patients compared to those under 65 years of age
Overdose Effects
- Reported effects: dizziness, drowsiness, fatigue, dry mouth
- Maximum tolerated dose not established
- Symptomatic and supportive treatment recommended
- Haemodialysis does not effectively remove Fexofenadine from blood
