Description
- A proton pump inhibitor used to relieve chronic heartburn symptoms, heal erosive esophagitis, control acid-related dyspepsia, treat duodenal and gastric ulcers, eradicate Helicobacter pylori infection, manage Zollinger-Ellison Syndrome, and relieve symptoms of GERD.
Indications
- Relief from chronic heartburn symptoms
- Healing of erosive esophagitis
- Maintenance of healing of erosive esophagitis
- Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
- Management of Zollinger-Ellison Syndrome
- Treatment of acid-related dyspepsia
- Treatment of duodenal and gastric ulcers
Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase in the gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Better acid control than racemic proton pump inhibitors
- Enteric-coated pellet formulation
- Absorption within 1.5 hours after oral administration
- Metabolized in the liver by the cytochrome P450 (CYP) enzyme system
- Excreted as inactive metabolites in the urine and faeces

- Oral dosage form for healing of erosive esophagitis
- Oral dosage form for symptomatic GERD
- Oral dosage form for Helicobacter pylori eradication
- Oral dosage form for Zollinger-Ellison Syndrome
- Oral dosage form for acid-related dyspepsia
- Oral dosage form for duodenal and gastric ulcer
- IV Injection for GERD with erosive esophagitis in adult patients
- IV Injection for risk reduction of rebleeding of gastric or duodenal ulcers in adult patients
- IV Injection for pediatric patients based on body weight
- IV Infusion for pediatric patients based on body weight
Administration
- Tablet or capsule should be swallowed whole and taken one hour before a meal
- Oral suspension should be mixed with water and consumed within 30 minutes
- IV Injection should be prepared using 0.9% Sodium Chloride and administered intravenously over a period of at least 3 minutes
Interaction
- Metabolized in the liver by CYP2C19 and CYP3A4
- Unlikely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1 and 3A4
- No clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin or amoxicillin
- Potential interference with drugs where gastric pH is an important determinant of bioavailability
- Increases plasma levels of certain drugs in combination therapy with clarithromycin
Contraindications
- Known hypersensitivity to any of the formulation

Pregnancy and lactation
- No adequate and well-controlled studies in pregnant women
- No teratogenic effects in animal studies
- Excretion in milk not studied
- Breast-feeding should be discontinued if use is essential
Precautions and warnings
- Symptomatic response does not preclude the presence of gastric malignancy
- Capsules should be taken at least one hour before meals
- Pellets inside capsules should not be chewed or crushed
- Antacids may be used while taking Esoral
Use in special populations
- Safety and effectiveness in paediatric patients not established
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
Overdose effects
- Signs of acute toxicity in animals
- No specific antidote known
- Treatment should be symptomatic and supportive

- Store at a temperature not exceeding 30°C
- Protect from light and moisture
- Keep out of reach of children
