Indications
- Relief of inflammatory manifestations
- Relief of pruritic manifestations
- Corticosteroid responsive eczema/dermatitis
Pharmacology
- Glucocorticoid with high topical anti-inflammatory potency
- Low HPA-axis suppressive activity after dermal administration
- Therapeutic index greater than most commonly available steroids
- High degree of selectivity for glucocorticoid receptor
- In vitro studies show strong affinity and agonist activity at human glucocorticoid receptors
- Weak affinity for progesterone receptor
- Virtually no affinity for mineralocorticoid, estrogen, or androgen receptors
- Therapeutic potency related to the half-life of the glucocorticoid-receptor complex (approximately 10 hours)
Dosage & Administration
- Cream: Apply a thin layer to affected skin areas once daily
- Ointment: Apply a thin layer to affected skin areas twice daily

Contraindications
- Rosacea
- Acne vulgaris
- Perioral dermatitis
- Primary cutaneous viral infections (e.g., Herpes simplex, chicken pox)
- Hypersensitivity to any of the ingredients
- Perianal and genital pruritus
- Not indicated in treatment of primarily infected skin lesions caused by fungi or bacteria
- Not indicated in children under one year of age
Side Effects
- Local burning
- Pruritus
- Signs of hypersensitivity
- Local atrophic changes in the skin (thinning, striae, dilatation of superficial blood vessels, hypertrichosis, hypopigmentation)
- Secondary infection
- Allergic contact dermatitis
- Exacerbation of the signs and symptoms of the dermatoses
- Features of hypercorticism with prolonged use of large amounts
- More likely to occur in infants and children, especially with occlusive dressings
Pregnancy & Lactation
- Consideration of use during pregnancy based on expected benefit vs. possible risk to the fetus
- Excretion into human breast milk not investigated
- Low plasma levels in patients following dermal application
- Therapeutic benefits vs. potential hazards in breastfeeding mothers and babies
Precautions & Warnings
- Very low propensity for systemic absorption
- Prolonged application of high doses to large areas might lead to adrenal suppression
- Children may absorb proportionally larger amounts
- Atropic changes on the face after prolonged treatment
- Appropriate antimicrobial therapy for treating infected inflammatory lesions
- Withdrawal and systemic administration of antimicrobial agents for spread of infection
- Encouragement of bacterial infection with occlusive dressing
Overdose Effects
- Acute overdosage unlikely
- Chronic overdosage or misuse may lead to features of hypercorticism
- Discontinuation of application in case of overdosage
- Extremely low oral bioavailability
