Indications
- Fluid retention associated with chronic congestive cardiac failure

Pharmacology
- Monosulphonyl diuretic
- Effective diuretic in low glomerular filtration rate (GFR)
- Distinctive action on renal tubular function
- Inhibition of sodium and chloride transport in the ascending limb of the loop of Henle
- Independence of action from acid-base balance changes
- Readily absorbed from the gastrointestinal tract
- Considerable proportions are bound to plasma proteins
- Rapid excretion in the urine
- Prompt onset of action
- Effect evident in about 5 minutes and lasts for about 2 hours
Dosage & Administration
- Adult dose of 40 mg daily
- Initial adult dose of 40 mg daily
- Reduced to 20 mg daily or 40 mg on alternate days
- Daily doses of 80 mg or higher may be required in some patients
- Usual initial dose of furosemide for hypertension is 80 mg
- Doses may be given in divided doses
- Maximum single dose should not exceed 600 mg/day
- Dosage titration required in the elderly
Interaction
- Marked fall in blood pressure when ACE inhibitors are added
- Increased serum lithium levels when given concomitantly
- Potent diuretics may increase toxic effects of nephrotoxic antibiotics
Contraindications
- Contraindicated in anuria
- Contraindicated in electrolyte deficiency
- Contraindicated in pre-comatose states associated with liver cirrhosis
- Hypersensitivity to furosemide or sulphonamides
Side Effects
- Disturbed electrolytes and water balance
- Prolonged use can produce alkalosis
- May cause uric acid retention
- May provoke hyperglycemia and glycosuria
Pregnancy & Lactation
- Pregnancy category C
- Caution during cardiogenic shock and in the first trimester of pregnancy
- Blood pressure and pulse monitoring during rapid diuresis
- Caution in patients liable to electrolyte deficiency
- Caution in nursing mothers
Precautions & Warnings
- Increased risk of developing acute retention in patients with prostatic hypertrophy
- Marked fall in blood pressure when ACE inhibitors are added
- Increased toxic effects of nephrotoxic antibiotics
- Adverse effects may impair the patient's ability to concentrate and react
Overdose Effects
- Clinical picture depends on the extent and consequences of electrolyte and fluid loss
- Signs and symptoms include hypovolaemia, dehydration, and cardiac arrhythmias
- No specific antidote known
- Measures such as gastric lavage or activated charcoal may be attempted
