Indications

  • Essential hypertension
  • Chronic congestive heart failure
  • Hepatic cirrhosis with collection of fluid in the abdominal cavity (ascites)
  • Swelling due to excess fluid retention (edema)
  • Hyperaldosteronism
  • Resistant edema associated with secondary hyperaldosteronism

Dosage & Administration

  • Furosemide 20 and spironolactone 50 mg dosage range: 20mg to 80mg of Furosemide and 50mg to 200mg of spironolactone daily
  • Furosemide 40 and spironolactone 50 mg dosage range: 40mg to 80mg of Furosemide and 50mg to 100mg of spironolactone daily
  • Not suitable for use in children
  • Excreted more slowly in the elderly

Interaction

  • Increased risk of hyperkalemia when taken with ACE inhibitors or potassium salts
  • Elevated levels of cardiac glycosides such as digoxin in the blood
  • Reduced or abolished diuretic effects when used with corticosteroids
  • Increased ototoxicity of aminoglycoside antibiotics when taken with Furosemide
  • Reduced natriuretic and anti-hypertensive effect when taken with sucralfate

Contraindications

  • Anuria
  • Acute renal insufficiency
  • Severe impairment of renal function
  • Hyperkalaemia
  • Addison's disease
  • Hypersensitivity to Spironolactone, Furosemide, or sulphonamides

Pregnancy & Lactation

  • Spironolactone and its metabolites may cross the placental barrier
  • Furosemide may cause fetal abnormalities
  • Use in pregnant women requires weighing benefits against possible hazards
  • Metabolites of Spironolactone detected in breast milk
  • Furosemide excreted in breast milk

Precautions & Warnings

  • Caution in patients liable to electrolyte deficiency
  • Use with caution in diabetes, enlarged prostate, hypotension, hypovolemia
