Indications
- Essential hypertension
- Chronic congestive heart failure
- Hepatic cirrhosis
- Swelling due to excess fluid retention
- Hyperaldosteronism
- Resistant edema associated with secondary hyperaldosteronism
Pharmacology
- Potassium-sparing diuretic
- Loop diuretic
- Complementary mechanisms
- Additive diuretic effect
- Synergistic diuretic effect
- Inhibits Na+/K+/2Cl- co-transporter
- Blocks reabsorption of sodium, potassium, and chloride ions
- Induces potassium loss
- Inhibits reabsorption of sodium in exchange for potassium
- Antagonizes the action of aldosterone
Dosage & Administration
- Furosemide 20 and spironolactone 50 mg
- 1 to 4 tablets daily
- 20 to 80 mg of Furosemide
- 50 to 200 mg of spironolactone
- Furosemide 40 and spironolactone 50 mg
- One to two tablets daily
- Furosemide 40 to 80 mg
- Spironolactone 50 to 100 mg
- Not suitable for use in children
Interaction
- Increased risk of hyperkalemia with ACE inhibitors or potassium salts
- Increases levels of cardiac glycosides such as digoxin in the blood
- May cause digitalis toxicity
- Corticosteroids may cause hypokalemia if used with Spironolactone
- Diuretic effects may be reduced when used with NSAIDs
- May increase the ototoxicity of aminoglycoside antibiotics
- Reduces the natriuretic and anti-hypertensive effect of Furosemide when used with sucralfate
Contraindications
- Anuria
- Acute renal insufficiency
- Severe impairment of renal function
- Hyperkalemia
- Addison's disease
- Hypersensitivity to Spironolactone, Furosemide or sulphonamides

- Breast enlargement
- Hirsutism
- Deepening of the voice
- Menstrual irregularities
- Impotence
- Transient increase in blood-urea-nitrogen concentrations
- Mild acidosis
- Hyponatremia
- Dehydration
- Reduction in blood volume
- Cardiac arrhythmias
Pregnancy & Lactation
- Crosses the placental barrier
- Possible hazards to the mother and fetus
- Furosemide may cause fetal abnormalities
- Metabolites detected in breast milk
- Alternative method of infant feeding should be instituted
- Excreted in breast milk
- Discontinue breast-feeding if treatment is essential
Precautions & Warnings
- Caution in patients liable to electrolyte deficiency
- Caution in diabetes
- Caution in enlarged prostate
- Caution in hypotension
- Caution in hypovolemia
