Indications
- Adjunct to diet and exercise for Type II diabetes mellitus
- Combination use with metformin for glycaemic control
- Combination use with insulin for hyperglycaemia control
- Potential for hypoglycaemia with combined use of Gipid and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Decreases blood glucose concentration
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Enhances glucose uptake
- Hypoglycaemic action persists for 24 hours
Dosage
- Governed by desired blood sugar level
- Lowest dose sufficient for metabolic control
- Based on regular monitoring of glucose levels
- Usual initial dose of 1 mg once daily
- Gradual dose titration intervals of 1 to 2 weeks
- Dose range in well-controlled diabetes: 1 to 4 mg daily
- Timing and distribution decided by physician
- Single daily dose before substantial breakfast or first main meal
- Dose adjustment with improved diabetes control and changes in weight or lifestyle
- Initial daily dose when switching from other oral antidiabetics: 1 mg
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, corticosteroids, fluconazole, MAO-inhibitors, etc.
- Attenuating drugs: Barbiturates, diuretics, phenothiazines, thyroid hormones, etc.
- Potential for masking warning symptoms of hypoglycaemic attack
- Unpredictable interaction with acute and chronic alcohol intake
Contraindications
- Not for insulin dependent (type I) diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Hypersensitivity to Glimepiride, sulfonylureas, sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea, vomiting, diarrhea, abdominal pain
- Urticaria, fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy; insulin changeover required
- Breast milk feeding may harm the child; not to be taken by breastfeeding women
Precautions & Warnings
- Increased risk of hypoglycaemia in initial weeks of treatment
- Necessity for careful monitoring and dose adjustment
- Prompt control of hypoglycaemia with immediate intake of carbohydrates
Overdose Effects
- Hypoglycaemia in cases of overdosage
- Mild symptoms treated with oral glucose and adjustments in drug dosage or meal patterns
- Severe symptoms constituting medical emergencies requiring hospitalization
- Treatment with rapid intravenous glucose injection
