Indications
- Lowering blood glucose in noninsulin dependent diabetes mellitus (NIDDM)
- Adjunct to diet and exercise for diabetes control
- Combination use with metformin
- Combination use with insulin for diabetes control
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates insulin release from pancreatic beta cells
- Improves beta cell sensitivity to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
Dosage
- Initial dose based on regular blood and urine glucose check
- Gradual dose titration based on monitoring
- Usual initial dose of 1 mg once daily
- Dose range for well controlled diabetes: 1 to 4 mg daily
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiating drugs include ACE inhibitors, fluconazole, chloramphenicol, and more
- Attenuating drugs include barbiturates, corticosteroids, diazoxide, and more
- Possible interactions with beta-receptor blockers, clonidine, and reserpine
Contraindications
- Not for insulin dependent diabetes mellitus
- Not for diabetic ketoacidosis or diabetic coma
- Contraindicated in patients hypersensitive to sulfonylureas or sulfonamides
- Not for patients with severe hepatic dysfunction or renal impairment

Pregnancy & Lactation
- Not to be taken during pregnancy, changeover to insulin necessary

Precautions & Warnings
- Initial weeks may have increased risk of hypoglycemia
- Necessity for careful monitoring and dosage adjustments
- Prompt control of hypoglycemia with immediate carbohydrate intake
Overdose Effects
- Overdosage can produce hypoglycemia
- Treatment with oral glucose for mild symptoms
- Severe cases require hospitalization and rapid IV glucose injection
