Indications
- Adjunct to diet and exercise for lowering blood glucose in patients with noninsulin dependent diabetes mellitus (NIDDM)
- Concomitant use with metformin when diet, exercise, or monotherapy does not provide adequate glycaemic control
- Use in combination with insulin to lower blood glucose in certain patients
- Increased potential for hypoglycaemia with combined use of Dialon and insulin
Pharmacology
- Sulfonylurea antidiabetic agent
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Enhances glucose uptake
- Hypoglycaemic action of a single dose persists for 24 hours
Dosage
- Initial dose of 1 mg once daily
- Gradual dose titration based on glucose monitoring
- Usual dose range in patients with well-controlled diabetes: 1 to 4 mg daily
- Timing and distribution of doses decided by the physician
- Avoid skipping meals after taking the drug
- Dose adjustment required with changes in weight or lifestyle
Administration
- Tablet must be swallowed with sufficient liquid
Interaction
- Potentiating drugs: ACE inhibitors, anabolic steroids, fluconazole, quinolones, sulphonamide antibiotics, etc.
- Attenuating drugs: Calcium channel blockers, corticosteroids, diuretics, nicotinic acid, etc.
- Use with beta-blockers, clonidine, guanethidine, or reserpine may mask hypoglycaemic symptoms
Contraindications
- Not suitable for insulin dependent diabetes mellitus or diabetic ketoacidosis
- Hypersensitivity to Glimepiride, sulfonylureas, or sulfonamides
- Severe hepatic dysfunction, severe impairment of renal function, and dialysis patients

- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy
- Changeover to insulin necessary
- Ingestion with breastfeeding may harm the child
- Not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycaemia
- Careful monitoring and dosage adjustments required
- Promptly controlled by intake of carbohydrates (glucose or sugar)
Overdose Effects
- Hypoglycaemia from overdosage
- Mild symptoms treated with oral glucose and dosage adjustments
- Severe reactions require immediate hospitalization
- Treatment with rapid intravenous injection of concentrated glucose solution
