Description

  • SB-Glim is an antidiabetic medication used to lower blood glucose levels in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM) who have hyperglycemia that cannot be controlled by diet and exercise alone. It can be used as an adjunct to diet and exercise, in combination with metformin or insulin, and may play a role in improving insulin sensitivity, glucose uptake, and reducing hepatic glucose production.

Active Ingredient

Mechanism of Action

  • Stimulates insulin release from functioning pancreatic beta cells, improves beta cell sensitivity to glucose stimulus, reduces basal hepatic glucose production, increases peripheral tissue sensitivity to insulin, and enhances glucose uptake.

Dosage Guidelines

  • Initial dose of 1mg once daily, with gradual stepwise titration based on regular blood sugar monitoring, usual dose range of 1 to 4mg daily, timing and distribution of doses to be decided by the physician based on the patient's lifestyle, dose adjustment with changes in weight or lifestyle, and changeover considerations from other oral antidiabetics.

Administration

  • Tablet must be swallowed with sufficient liquid

Interaction with Other Drugs

  • Potentiation of hypoglycemic action by ACE inhibitors, steroids, fluconazole, salicylates, and many other drugs, and potential attenuation by barbiturates, calcium channel blockers, corticosteroids, and others.

Contraindications

  • Not suitable for insulin dependent (Type I) diabetes mellitus, diabetic ketoacidosis, hypersensitivity to glimepiride or sulfonylureas, severe hepatic dysfunction, severe impairment of renal function, and dialysis patients.

Side Effects

  • Hypoglycemia, temporary visual impairment, gastrointestinal symptoms, urticaria, and changes in blood pressure.

Pregnancy & Lactation

  • Not to be taken during pregnancy, requires changeover to insulin, and must not be taken during breastfeeding.

Precautions & Warnings

  • Increased risk of hypoglycemia in initial weeks of treatment requiring careful monitoring and potential adjustment of dosage, prompt management of hypoglycemia with immediate intake of carbohydrates.

Overdose Effects

  • Potential to produce hypoglycemia which may require aggressive treatment with oral glucose or intravenous infusion, and close monitoring until the patient is out of danger.
