Indications
- Relief from chronic heartburn symptoms and other symptoms associated with GERD

Pharmacology
- Proton pump inhibitor
- Suppresses gastric acid secretion
- Specific inhibition of the H+/K+-ATPase
- Peak plasma levels occur at approximately 1.5 hours
- Distribution: 97% bound to plasma proteins
- Metabolized in the liver by the cytochrome P450 enzyme system
- Plasma elimination half-life of approximately 1–1.5 hours
- Combination therapy with antimicrobials
- AUC increased by 70% and Cmax by 18% during triple combination therapy
- Not likely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1, and 3A4
Dosage
- Healing of Erosive Esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
- Maintenance of Healing of Erosive Esophagitis: 20 mg Once Daily
- Symptomatic GERD: 20 mg Once Daily for 4 Weeks
- Helicobacter Pylori eradication: Triple Therapy - Esomeprazole 40 mg Once Daily for 10 days
- Zollinger-Ellison Syndrome: 20-80 mg once daily
- Acid-related Dyspepsia: 20-40 mg once daily for 2-4 weeks
- Duodenal ulcer: 20 mg once daily for 2-4 weeks
- Gastric ulcer: 20-40 mg once daily for 4-8 weeks
- IV Injection: 20 mg or 40 mg once daily
- IV Infusion for adults and pediatric patients
Administration
- Tablet or capsule: should be swallowed whole and taken one hour before a meal
- Oral Suspension: contents should be taken into a glass containing 15 ml of water
- IV Injection: administered intravenously over a period of at least 3 minutes
Interaction
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- No clinically significant interactions with phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin
- May interfere with the absorption of drugs where gastric pH is important for bioavailability
Contraindications
- Contraindicated in patients with known hypersensitivity to any of the formulation

Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Animal studies have revealed no teratogenic effects
- Excretion of esomeprazole in milk has not been studied
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Capsules should be taken at least one hour before meals
- Antacids may be used while taking Maxima
- Information for patients regarding capsule administration
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- No overall differences in safety and efficacy observed between the elderly and younger individuals
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
Overdose Effects
- Single oral dose lethal to rats at 510 mg/kg
- Major signs of acute toxicity: reduced motor activity, changes in respiratory frequency, tremor, ataxia, intermittent clonic convulsions
- No specific antidote known
- Treatment should be symptomatic and supportive

- Store at a temperature not exceeding 30°C in a dry place
- Protect from light and moisture
- Keep out of reach of children
