Indications

  • Treatment of Duodenal & Gastric ulcers

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Specific inhibition of H+/K+-ATPase
  • First single optical isomer of proton pump inhibitor
  • Better acid control than racemic proton pump inhibitors
  • Enteric-coated pellet formulation
  • Peak plasma levels occur at approximately 1.5 hours
  • Systemic bioavailability is approximately 90%

Dosage

  • Oral dosage form
  • Healing of erosive esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
  • Maintenance of healing of erosive esophagitis: 20 mg Once Daily
  • Symptomatic GERD: 20 mg Once Daily for 4 Weeks
  • Helicobacter Pylori eradication: Triple Therapy
  • Zollinger-Ellison Syndrome: 20-80 mg once daily
  • Acid-related Dyspepsia: 20-40 mg once daily
  • Duodenal ulcer: 20 mg once daily for 2-4 weeks
  • Gastric ulcer: 20-40 mg once daily for 4-8 weeks
  • IV Injection or IV infusion for various conditions and patient age groups

Administration

  • Tablet or capsule should be swallowed whole and taken one hour before a meal

Interaction

  • Extensive metabolism in the liver by CYP2C19 and CYP3A4
  • Inhibition of gastric acid secretion
  • Potential interference with CYP2C19
  • No clinically significant interactions with various drugs

  • Known hypersensitivity to any of the formulation

  • Symptomatic response does not preclude the presence of gastric malignancy
  • Specific instructions for patients with difficulty swallowing capsules
  • Use of antacids while taking Nexe

Use in Special Populations

  • Safety and effectiveness in paediatric patients not established
  • No overall differences in safety and efficacy in the elderly
  • No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
  • No alteration of pharmacokinetics in patients with renal impairment

Overdose Effects

  • Lethal to rats at a high dose
  • Signs of acute toxicity
  • No specific antidote known
  • Symptomatic and supportive treatment recommended
