Pharmacology
- Proton pump inhibitor
- Specific inhibition of H+/K+-ATPase in gastric parietal cell
- First single optical isomer of proton pump inhibitor
- Better acid control than racemic proton pump inhibitors
- Enteric-coated pellet formulation of esomeprazole magnesium
- Peak plasma levels occur at approximately 1.5 hours
- Distribution: 97% bound to plasma proteins
- Metabolized in the liver by CYP enzyme system
- Plasma elimination half-life of approximately 1-1.5 hours
Dosage
- Oral dosage form
- Healing of Erosive Esophagitis: 20mg or 40mg Once Daily for 4-8 Weeks
- Symptomatic GERD: 20mg Once Daily for 4 Weeks
- IV Injection: 20mg or 40mg once daily for adult patients
- Maintenance of healing of erosive esophagitis: 20mg Once Daily
- Treatment for Helicobacter pylori eradication: 40mg Once Daily for 10 days in combination therapy
- Zollinger-Ellison Syndrome: 20-80mg once daily
- Acid-related Dyspepsia: 20-40mg once daily for 2-4 weeks
- Dosing in pediatric patients based on weight
Administration
- Tablet or capsule to be swallowed whole
- Oral suspension to be taken with water
- IV Injection: must be administered intravenously over a period of at least 3 minutes
Interactions
- Extensively metabolized in the liver by CYP2C19 and CYP3A4
- No clinically relevant interactions with certain CYP enzymes expected
- Potential interference with absorption of drugs where gastric pH is important for bioavailability
- Interaction with oral contraceptives, diazepam, phenytoin, or quinidine
Contraindications
- Contraindicated in patients with known hypersensitivity to any of the formulation
Side Effects
- Frequently occurring adverse events include headaches, diarrhea, nausea, flatulence, abdominal pain, constipation, and dry mouth
Pregnancy & Lactation
- No adequate and well-controlled studies in pregnant women
- Animal studies have revealed no teratogenic effects
- Excretion of esomeprazole in milk not studied
Precautions & Warnings
- Symptomatic response to therapy does not preclude the presence of gastric malignancy
- Administration at least one hour before meals
- Instruction for patients who have difficulty swallowing capsules
Use in Special Populations
- Safety and effectiveness in pediatric patients not established
- No overall differences in safety and efficacy for geriatric use
- No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
- Pharmacokinetics not expected to be altered in patients with renal impairment
Overdose Effects
- No specific antidote known
- Treatment should be symptomatic and supportive
- Possibility of multiple drug ingestion should be considered
