Indications

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Inhibition of H+/K+-ATPase
  • Better acid control
  • Enteric-coated pellet formulation
  • Peak plasma levels at approximately 1.5 hours
  • Metabolized in the liver by CYP enzymes
  • Extensive metabolism in the liver
  • Plasma protein binding

Dosage

  • 20 mg or 40 mg Once Daily for healing of erosive esophagitis
  • 20 mg Once Daily for maintenance of healing of erosive esophagitis
  • 20 mg Once Daily for symptomatic GERD
  • Triple Therapy for Helicobacter pylori eradication
  • 20-80 mg Once Daily for Zollinger-Ellison Syndrome
  • 20-40 mg Once Daily for acid-related Dyspepsia
  • 20 mg Once Daily for 2-4 weeks for Duodenal ulcer
  • 20-40 mg Once Daily for 4-8 weeks for Gastric ulcer
  • Dosage for IV Injection or IV infusion

Administration

  • Swallowed whole and taken one hour before a meal for tablets or capsules
  • Instructions for oral suspension
  • Preparation of IV Injection solution
  • Clear and colorless to very slightly yellow solution

Interaction

  • Extensive metabolism in the liver by CYP2C19 and CYP3A4
  • No clinically relevant interactions with certain CYP enzymes
  • Potential interference with the absorption of certain drugs
  • Co-administration with oral contraceptives, diazepam, phenytoin, or quinidine

Contraindications

  • Known hypersensitivity to any of the formulation

Pregnancy & Lactation

  • No adequate and well-controlled studies in pregnant women

  • Excretion of esomeprazole in milk not studied
  • Discontinuation of breastfeeding if essential use of esomeprazole

  • Symptomatic response does not preclude the presence of gastric malignancy

Use in Special Populations

  • Safety and effectiveness in paediatric patients not established
  • No overall differences in safety and efficacy in elderly
  • No dosage adjustment for patients with mild to moderate hepatic insufficiency
  • No alteration of pharmacokinetics in patients with renal impairment

Overdose Effects

  • Acute toxicity in animal studies
  • No specific antidote known
  • Expected to be extensively protein bound
  • Symptomatic and supportive treatment recommended
