Indications

  • Relief from chronic heartburn symptoms and other symptoms associated with GERD
  • Healing of erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Eradication of Helicobacter pylori infection in patients with duodenal ulcer disease
  • Treatment of Zollinger-Ellison Syndrome
  • Treatment of acid related Dyspepsia
  • Treatment of duodenal and gastric ulcers

Pharmacology

  • Proton pump inhibitor
  • Suppression of gastric acid secretion
  • Inhibition of H+/K+-ATPase in the gastric parietal cell
  • Better acid control than racemic proton pump inhibitors
  • Enteric-coated pellet formulation of esomeprazole magnesium

Dosage

  • 20 mg or 40 mg Once Daily for 4-8 Weeks for healing of erosive esophagitis
  • 20 mg Once Daily for maintenance of healing of erosive esophagitis
  • 20 mg Once Daily for symptomatic GERD for 4 Weeks
  • Triple Therapy for Helicobacter Pylori eradication
  • 20-80 mg once daily for Zollinger-Ellison Syndrome
  • 20-40 mg once daily for acid-related Dyspepsia
  • 20 mg once daily for 2-4 weeks for duodenal ulcer
  • 20-40 mg once daily for 4-8 weeks for gastric ulcer
  • Dosage for IV Injection or IV infusion for various conditions and age groups

Administration

  • Swallow whole and taken one hour before a meal for tablets or capsules
  • Preparation of oral suspension and administration through nasogastric or gastric tube
  • Preparation and administration of IV Injection solution
  • Specific instructions for administration with applesauce

Interaction

  • Extensively metabolized by CYP2C19 and CYP3A4 in the liver
  • No clinically relevant interactions with certain drugs
  • Potential interference with drugs affected by gastric pH

Contraindications

  • Known hypersensitivity to any of the formulation

  • Breast-feeding should be discontinued if essential

Precautions & Warnings

  • Symptomatic response to therapy does not preclude the presence of gastric malignancy
  • Specific instructions for patients regarding administration and use of antacids

Use in Special Populations

  • Safety and effectiveness in paediatric patients not established
  • No overall differences in safety and efficacy in geriatric use
  • No dosage adjustment recommended for mild to moderate hepatic insufficiency
  • No alteration in Pharmacokinetics expected in patients with renal impairment

Overdose Effects

  • Lethal effects observed in animal studies
  • No reports of overdose in humans
  • No specific antidote known
  • Symptomatic and supportive treatment recommended
