Description

Sergel is a proton pump inhibitor used to relieve chronic heartburn symptoms, heal erosive esophagitis, eradicate Helicobacter pylori infection, treat Zollinger-Ellison Syndrome, acid-related Dyspepsia, and duodenal & gastric ulcers.

Pharmacology

  • Mode of action: Suppresses gastric acid secretion by specific inhibition of the H+/K+-ATPase
  • Absorption: Enteric-coated pellet formulation with peak plasma levels occurring at approximately 1.5 hours
  • Distribution: 97% bound to plasma proteins; apparent volume of distribution at steady state in healthy volunteers is approximately 16 L
  • Metabolism: Extensively metabolized in the liver by the cytochrome P450 (CYP) enzyme system
  • Excretion: Plasma elimination half-life of approximately 1-1.5 hours; less than 1% of parent drug excreted in the urine

Dosage

  • Oral:
    • Healing of erosive esophagitis: 20 mg or 40 mg Once Daily for 4-8 Weeks
    • Maintenance of healing of erosive esophagitis: 20 mg Once Daily
    • Symptomatic gerd: 20 mg Once Daily for 4 Weeks
    • Helicobacter pylori eradication: 40 mg Once Daily for 10 days
    • Zollinger ellison syndrome: 20-80 mg once daily
    • Acid related dyspepsia: 20-40 mg once daily for 2-4 weeks
    • Duodenal ulcer: 20 mg once daily for 2-4 weeks
    • Gastric ulcer: 20-40 mg once daily for 4-8 weeks
  • Iv injection:
    • Gerd with erosive esophagitis: 20 mg or 40 mg once daily for adult patients; dosage adjustments for pediatric patients
  • Risk reduction of rebleeding of gastric or duodenal ulcers: 80 mg IV Infusion for adults

Administration

  • Tablet or capsule: Swallowed whole and taken one hour before a meal
  • Oral suspension: Mixed with water, stirred well, and should be taken immediately; for administration through a tube, specific instructions apply
  • Iv injection: Administered intravenously over a period of at least 3 minutes

Interaction

  • Metabolism: Extensively metabolized in the liver by CYP2C19 and CYP3A4
  • Interference with absorption: May interfere with the absorption of drugs where gastric pH is an important determinant of bioavailability
  • Co-administration impacts: Potential increases in the plasma levels of certain drugs when co-administered
