Indications

• Relief from chronic heartburn symptoms
• Healing of erosive esophagitis
• Maintenance of healing of erosive esophagitis
• Eradication of Helicobacter pylori infection
• Treatment of Zollinger-Ellison Syndrome
• Treatment of acid-related Dyspepsia
• Treatment of duodenal and gastric ulcers

Pharmacology

• Proton pump inhibitor
• Suppression of gastric acid secretion
• Specific inhibition of the H+/K+-ATPase
• First single optical isomer of proton pump inhibitor
• Better acid control than racemic proton pump inhibitors
• Enteric-coated pellet formulation
• Peak plasma levels occur at approximately 1.5 hours
• Systemic bioavailability is approximately 90%
• Extensively metabolized in the liver
• Metabolism is dependent upon the CYP2C19 and CYP3A4 enzymes

Dosage

• Oral dosage form available
• Dosage for healing of erosive esophagitis
• Dosage for symptomatic GERD
• Dosage for Helicobacter pylori eradication
• Dosage for Zollinger-Ellison Syndrome
• IV injection or infusion dosage for different patient groups
• Dosage for risk reduction of rebleeding of ulcers following therapeutic endoscopy

Administration

• Tablet or capsule should be swallowed whole

Interaction

• Metabolized in the liver by CYP2C19 and CYP3A4
• Not likely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1 and 3A4
• Clinically significant interactions with certain drugs
• Potential interference with drug absorption

Contraindications

• Known hypersensitivity to any of the formulation

Pregnancy & Lactation

• No adequate and well-controlled studies in pregnant women
• No teratogenic effects observed in animal studies

• Symptomatic response does not preclude the presence of gastric malignancy
• Instructions for patients regarding capsule administration
• Use of antacids while taking Xelpro
• Use in paediatric, geriatric, hepatic, and renal special populations

Use in Special Populations

• Safety and effectiveness not established in paediatric patients
• No overall differences in safety and efficacy observed in geriatric patients
• No dosage adjustment recommended for patients with mild to moderate hepatic insufficiency
• Pharmacokinetics not expected to be altered in patients with renal impairment

Overdose Effects

• Acute toxicity signs and symptoms observed in animal studies
• No specific antidote known
• Treatment should be symptomatic and supportive

Storage Conditions

• Store at a temperature not exceeding 30°C
• Protect from light and moisture
• Keep out of reach of children
