Indications

Mode of Action
- Non-steroidal anti-inflammatory drug (NSAID)
- Analgesic
- Antipyretic
- Inhibition of prostaglandin synthesis
- Stabilization of lysosomal membranes
- Inhibition of leukotriene synthesis
- Antibradykinin activity
- Peripheral and central prostaglandin synthesis inhibition
Dosage
- Anti-inflammatory dosage 150 to 300 mg/day
- Maintenance dosage 100 to 200 mg/day
- Maximum daily dose 300 mg
- Management of pain and primary dysmenorrhea 25 to 50 mg every 6 to 8 hours
- Total daily dose not to exceed 300 mg
Administration
- Oral forms to be taken with fluids
- Preferably with food
Interactions
- Increased risk of gastrointestinal ulceration and bleeding with other NSAIDs
- Increased risk of bleeding with anticoagulants
- Coadministration requires close patient monitoring
- Risk of elevated lithium plasma levels with lithium
- Increased risk of haematologic toxicity of methotrexate at high doses
- Risk of hyperkalemia with certain medicinal products and therapeutic categories
- Increased risk of gastrointestinal ulceration or bleeding with corticosteroids
- Greater risk of developing renal failure with diuretics
Contraindications
- History of hypersensitivity reactions
- Severe heart failure
- Active or history of peptic ulcer/hemorrhage
- Severe hepatic or renal insufficiency
- Third trimester of pregnancy
Side Effects
- Haemorrhagic anaemia
- Agranulocytosis
- Thrombocytopenia
- Bone marrow failure
- Dizziness
- Vision blurred
- Elevation of liver enzymes
- Rash
- Pruritis
- Renal function tests abnormal
Pregnancy & Lactation
- Avoid use during first and second trimester
- Contraindicated in the last trimester
- Not recommended in nursing mothers
Precautions & Warnings
- Given with care to patients with a history of gastrointestinal disease
- Renal function monitoring in certain patients
- Caution in patients with abnormal liver function tests
- Possible impairment of female fertility
Use in Special Populations
- Elderly: Reduce initial dosage
- Hepatic impairment: Monitor and keep at minimal effective daily dosage
- Renal impairment: Reduce initial dosage
Overdose Effects
- Lethargy
- Drowsiness
- Nausea
- Vomiting
- Epigastric pain
- Symptomatic and supportive treatment for overdose
