Indications
- Short-term management of moderate to severe acute post-operative pain
- Relief of pain, inflammation, and light sensitivity after ocular surgery
- Decrease in eye inflammation or redness caused by seasonal allergies
Description
- Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drug (NSAID)
- Chemical composition: 5 benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid, compound with 2 - amino-2-(hydroxymethyl)-1,3-propanediol (1:1)
- Inhibits prostaglandin synthesis
- Biological activity associated with the S-form
- Linear pharmacokinetic property
- Highly protein bound
- Largely metabolized in the liver
- Metabolites and some unchanged drug excreted in urine
Pharmacology
- Potent analgesic of the non-steroidal anti-inflammatory drugs (NSAIDs)
- Inhibits cyclooxygenase enzyme system
- Minimal anti-inflammatory effect at analgesic dose
Dosage & Administration
- Tablet recommended dose: 10 mg every 4-6 hours
- Tablet not recommended for chronic use
- Injection may be used for moderately severe, acute pain management
- Recommended IV bolus administration over at least 15 seconds
- Recommended IM administration slowly and deeply into the muscle
- Onset of analgesic effect within 30 minutes (IV or IM)
- Maximum analgesic effect in 1 to 2 hours (IV or IM)
- Duration of analgesic effect usually 4 to 6 hours
- Different dosing for various age groups and conditions
- Eye Drops: 1 drop in each eye 4 times daily
Interaction
- Other NSAIDs or aspirin: Increase side effects of KTL
- Anti-coagulants: Enhance anti-coagulant effect
- Beta Blocker: Reduce anti-hypertensive effect
- ACE Inhibitors: Increase risk of renal impairment
- Methotrexate: Enhance toxicity of methotrexate
Contraindications
- Hypersensitivity to KTL or other NSAIDs
- Not to be used in children under 16 years of age
- Contraindicated as prophylactic analgesic before surgery

Precautions & Warnings
- Caution in patients over the age of 65 years
- Caution in patients with active or suspected peptic ulcer, gastrointestinal bleeding, asthma, and liver dysfunction
- Discontinue use of topical NSAIDs and monitor for corneal health in patients with corneal epithelial breakdown
- Avoid administration while wearing contact lenses
