Indications

  • Short term treatment of active duodenal ulcer
  • Maintenance of healed duodenal ulcers
  • Short term treatment of active benign gastric ulcers
  • Short term treatment of active erosive esophagitis
  • Maintenance of healing of erosive esophagitis
  • Pathological hypersecretory conditions including Zollinger- Ellison Syndrome
  • H. pylori eradication to reduce the risk of duodenal ulcer recurrence

Pharmacology

  • Substituted benzimidazole
  • PPI (Proton Pump Inhibitor)
  • Inhibition of H+/K+ ATPase enzyme system in gastric parietal cell

Dosage & Administration

  • 30 mg daily in the morning for benign gastric ulcer
  • 30 mg daily in the morning for 4 weeks for duodenal ulcer
  • 15-30 mg daily for NSAID-associated duodenal or gastric ulcer
  • Initially 60 mg once daily for Zollinger-Ellison syndrome; adjusted according to response
  • 30 mg daily in the morning for gastroesophageal reflux disease
  • 15-30 mg daily in the morning for acid-related dyspepsia

Interaction

  • Selective inhibitor of cytochrome P-450 monooxygenase system
  • Uncertainty over the effect on the oral combined contraceptive pill
  • Reduction in gastric acid influencing bacterial colonization of the stomach and duodenum
  • Potential influence on vitamin B12 absorption

Contraindications

  • Known hypersensitivity to any component of the formulation

Side Effects

  • Possible induction of carcinoid tumors by profound acid suppression
  • Rise in serum gastrin levels
  • Accelerated development of atrophic gastritis in long term treatment with a proton pump inhibitor in patients with Helicobacter pylori infection
  • Dose dependent diarrhea and headache

Pregnancy & Lactation

  • Avoided in pregnancy unless compelling reasons

  • Rule out gastric malignancy
  • Hepatic impairment
  • Pregnancy and lactation

Use in Special Populations

  • Not recommended for neonates
  • Clinical use in children as young as 13 years old
  • No increase in adverse drug reactions in the elderly
