Indications

  • Management of moderate to severe acute post-operative pain
  • Relief of pain, inflammation, and light sensitivity after ocular surgery
  • Treatment of eye inflammation or redness caused by seasonal allergies

Description

  • Belongs to pyrrolo-pyrrole group of nonsteroidal anti-inflammatory drugs (NSAIDs)
  • Chemical name: 5-benzoyle-2,3-dihydro-1H-pyrroligine-1-carboxylic acid
  • Biological activity associated with S-form
  • Highly protein bound
  • Metabolized in the liver
  • Metabolites and some unchanged drugs excreted in urine

Pharmacology

  • Potent analgesic
  • Inhibits cyclooxygenase enzyme system
  • Minimal anti-inflammatory effect at analgesic dose

Dosage & Administration

  • Tablet recommended dose: 10 mg every 4-6 hours for up to 7 days
  • Injection for postoperative pain management: IV or IM administration
  • Eye Drops: 1 drop in each eye 4 times daily
  • Different doses for adults vs. patients under 16 years of age
  • Max daily doses for different age groups

Interactions

  • Enhanced side effects with other NSAIDs or aspirin
  • Enhanced anti-coagulant effect with anti-coagulants
  • Reduced anti-hypertensive effect with beta blockers
  • Increased risk of renal impairment with ACE Inhibitors
  • Enhanced toxicity of methotrexate

Contraindications

  • Hypersensitivity to the drug or other NSAIDs
  • Use not recommended in children under 16 years of age
  • Contraindicated as prophylactic analgesic before surgery

  • Discontinue use and monitor closely in patients with corneal epithelial breakdown
