Indications
- Hypertension

Pharmacology
- Adrenergic receptor-blocking agent
- Selective alpha 1-adrenergic blocking action
- Nonselective beta-adrenergic blocking action
- Decrease in blood pressure
- Peak plasma levels after 1 to 2 hours of oral administration
- Plasma half-life of about 6 to 8 hours
- Approximately 55% to 60% excreted in urine within 24 hours
Dosage & Administration
- Initial dosage of 100 mg twice daily for adults
- Maintenance dosage between 200 and 400 mg twice daily
- Severe hypertension may require 1,200 to 2,400 mg per day
- Intravenous injection for rapidly reducing blood pressure
- Intravenous infusion for hypertension in pregnancy, following acute myocardial infarction, after stroke, and hypotensive anaesthesia
Interaction
- Enhanced hypotensive effects with halothane
- Potential interaction with Class I antiarrhythmic agents
- Possible interaction with calcium antagonists of the verapamil type
- Reduction of hypotensive effect when used with prostaglandin synthetase inhibitors
- Increased incidence of tremor with tricyclic antidepressants
- Potential increase in bioavailability with cimetidine
Contraindications
- Hypersensitivity to the medicine
- Second or third degree heart block
- Infranodal A-V block
- Uncontrolled heart failure
- Sick-sinus syndrome
- Cardiogenic shock
- Severe and prolonged hypotension or severe bradycardia
- Bronchial asthma or other obstructive lung disorders

Precautions & Warnings
- Rare reports of severe hepatocellular injury
- Reversible hepatic injury
- Caution in patients with hepatic impairment
- Caution in patients with peripheral vascular disease
Overdose Effects
- Excessive hypotension
- Excessive bradycardia
- Gastric lavage for removal of the drug
- Atropine for excessive bradycardia
- Digitalis glycoside and diuretic for cardiac failure
- Vasopressors for hypotension
- Epinephrine for bronchospasm
