Indications
- Epilepsy- adjunctive therapy in patients aged 2 years and older
- Partial-onset seizures
- Primary generalized tonic-clonic seizures
- Generalized seizures of Lennox-Gastaut syndrome
- Epilepsy- monotherapy in patients aged 16 years and older
- Bipolar disorder- patients aged 18 years and older
Description
Phenyltriazine antiepileptic used to treat epilepsy and bipolar disorder. Controls seizures by inhibiting voltage-sensitive sodium channels and stabilizing neuronal membranes.
Mode of Action
Inhibits voltage-sensitive sodium channels, modulates release of presynaptic excitatory neurotransmitters, weakly binds to various receptors, inhibits Cav2.3 (R-type) calcium currents.
Dosage & Administration
- Week-based escalation regimen for epilepsy in patients older than 12 years
- Usual maintenance doses for different patient groups
- Initial weight-based dosing guide for patients aged 2 to 12 years taking Valproate
- Escalation regimen for bipolar disorder in adults
Interaction
- Valproate increases concentrations by more than 2-fold
- Carbamazepine, phenytoin, phenobarbital, primidone, and rifampin decrease concentrations by approximately 40%
- Estrogen-containing oral contraceptives decrease concentrations by approximately 50%
- Protease inhibitors decrease exposure by approximately 32-50%

Overdose Effects
