Description
- Proton Pump Inhibitor
- Suppresses gastric acid production
- Treats peptic ulcer diseases, GERD, ulcer induced by NSAIDs, eradication of Helicobacter pylori, Zollinger-Ellison Syndrome
- Well tolerated in both short-term and long-term treatment
- Common side effects: headache, diarrhea, abdominal pain, flatulance, rash, insomnia, hyperglycemia
- FDA pregnancy category B
- Caution: tablet should not be split, chewed, or crushed
- May lead to malabsorption of Vitamin B12
- May increase the risk of osteoporosis related disorders
- No significant drug interactions observed in clinical studies
- Contraindicated in patients with known hypersensitivity to any of the components of the formulation
- Storage: Keep in a dry place, away from light and heat, out of the reach of children

Indications
- Peptic ulcer diseases
- Gastroesophageal reflux diseases
- Ulcer induced by non-steroidal anti-inflammatory drugs (NSAIDs)
- Eradication of Helicobacter pylori (in combination with antibiotics)
- Zollinger-Ellison Syndrome
Pharmacology
- Proton pump inhibitor
- Suppresses final step in gastric acid production
- Covalently binds to the H+/K+ ATPase enzyme system at the surface of the gastric parietal cell
- Leads to inhibition of both basal and stimulated gastric acid secretion
- Irrespective of the stimulus, effect persists longer than 24 hours
Dosage administration
- Oral dosage for benign gastric ulcer: 40 mg daily in the morning for 4 weeks, may continue for further 4 weeks if not fully healed
- Oral dosage for gastro-esophageal reflux disease: 20-40 mg daily in the morning for 4 weeks, may continue for further 4 weeks if not fully healed; maintenance dose is 20 mg daily, may be increased to 40 mg daily
- Intravenous dosage for duodenal ulcer and gastric ulcer: 40 mg once daily for 7-10 days
- Intravenous dosage for prevention of rebleeding in peptic ulcer: IV 80 mg, followed by 8 mg/hour infusion for 72 hours

Contraindications
- Contraindicated in patients with known hypersensitivity to any of the components of the formulation

Pregnancy lactation
- FDA pregnancy category B
- No adequate and well-controlled studies in pregnant women
- Excreted in human milk
- Decision should be made whether to discontinue nursing or to discontinue the drug, taking into account the benefit of the drug to the mother
Precautions warnings
- Tablet should not be split, chewed, or crushed
- Long-term therapy may lead to malabsorption of cyanocobalamin (Vitamin B12)
- May increase the risk of osteoporosis related disorders
Overdose effects
- No known symptoms of overdosage in humans
Reconstitution
- IV injection should be given as a slow intravenous injection
- Reconstituted solution may be stored at room temperature for a maximum of 4 hours
- IV infusion should be given as an intravenous infusion over a period of approximately 15 minutes
- Reconstituted solution may be stored at room temperature for a maximum of 4 hours prior to further dilution
