Indications
- Local anaesthetic for infiltration blockade and intravenous regional analgesia
- Prevention and treatment of ventricular tachyarrhythmias
Description
Sterile, non-pyrogenic, aqueous solution containing Chemocain, a local anaesthetic of the amide type with fast onset and intermediate duration of action
Pharmacology
Acts by inhibiting sodium influx through sodium specific ion channels in the neuronal cell membrane, particularly voltage-gated sodium channels
Dosage
- Adult: Maximum individual dose should not exceed 4.5mg/kg of body weight, maximum total dose should not exceed 300 mg
- Infiltration:
- Percutaneous: Total dose 5-300 mg
- Intravenous regional: Total dose 50-300 mg
- Peripheral Nerve Blocks:
- Brachial: Total dose 225-300 mg
- Dental: Total dose 20-100 mg
- Intercostal: Total dose 30 mg
- Paravertebral: Total dose 30-50 mg
- Pudenal:
- Paracervical Obstetrical analgesia: Total dose 100 mg
- Sympathetic Nerve Blocks:
- Cervical: Total dose 50 mg
- Lumbar: Total dose 50-100 mg
- Central Neural Blocks:
- Epidural: Total dose 200-300 mg [Dose determined by number of dermatomes to be anesthetized (2-3 mL/dermatome) periodically.]
- Thoracic: Total dose 200-300 mg
Administration
Intravenous dose of 1 mg.kg-1 may be given over 2 min and repeated after 5 min
Interaction
Cimetidine can impair the metabolism of lidocaine absorbed into the circulation, beta blockers such as propranolol, metoprolol or nadolol can increase plasma lidocaine concentrations, suxamethonium action duration is prolonged
Contraindications
Known hypersensitivity to anaesthetics of the amide type
Side Effects
- Adverse reactions are rare, may involve central nervous system and/or cardiovascular system, allergic reactions are extremely rare
Pregnancy & Lactation
Not recommended during early pregnancy unless benefits outweigh risks, caution in nursing women
Precautions & Warnings
Should be used cautiously in patients with epilepsy, impaired cardiac conduction, bradycardia, impaired respiratory function, and impaired hepatic function
Use in Special Populations
Dosages in children should be reduced, elderly patients may be more sensitive to systemic effects
