Indications
- Treatment of hypertension
- Renal protection in type-2 diabetic patients with proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose of 50 mg
- Usage alone or in combination with other antihypertensive agents
- Dosing adjustment for patients with intravascular volume-depletion
- Different dosage recommendation for patients over 75 years
Interaction
- Reduction in levels by rifampicin and fluconazole
- Potentiation of effects with hydrochlorothiazide
- Increases in serum potassium with potassium-sparing diuretics
- Attenuation of effects by indomethacin
- Increased risk of renal impairment with concurrent use of certain drugs
Contraindications
- Contraindicated in pregnant women
- Contraindicated in patients hypersensitive to any components
- Should not be administered with Aliskiren in diabetic patients
Side Effects
- Mild and transient side effects
- Common side effects include dizziness, diarrhea, nasal congestion, cough
- Other side effects such as fatigue, edema, abdominal pain, chest pain
Pregnancy & Lactation
- Pregnancy Category D
- Increased fetal risk if administered during second or third trimester
- Unknown excretion in human milk
Precautions & Warnings
- Reduction in fetal renal function and increase in fetal and neonatal morbidity and death
- Risk of symptomatic hypotension in intravascularly volume-depleted patients
- Significant increase in plasma concentration in cirrhotic patients
- Reported changes in renal function and renal failure in renal impaired patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Lower starting dose for patients with moderate to severe renal impairment or on dialysis
- Consideration of lower dose for patients with hepatic impairment
- Contraindicated in pregnancy and in patients hypersensitive to losartan
