Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- Non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues
- Reduces vasoconstriction and aldosterone release
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- May be used alone or in combination with other antihypertensive agents
- Total daily dose ranges from 25 mg to 100 mg
Interaction
- Reduced levels with rifampicin and fluconazole
- Potentiation with concomitant use of hydrochlorothiazide
- Possible increase in serum potassium with potassium-sparing diuretics
- Attenuation by non-steroidal anti-inflammatory drug indomethacin
Contraindications
- Contraindicated in pregnant women
- Hypersensitivity to any component of the product
- Should not be administered with Aliskiren in patients with diabetes
Side Effects
- Common side effects include dizziness, diarrhea, nasal congestion, cough, and upper respiratory infection
- Other side effects include fatigue, edema, abdominal pain, chest pain, nausea, headache, and pharyngitis
Pregnancy & Lactation
- Pregnancy Category D
- Risk increases if administered during second or third trimesters of pregnancy
- Discontinuation of nursing or drug should be decided on importance to the mother
Precautions & Warnings
- Reduces fetal renal function and increases fetal and neonatal morbidity and death during second and third trimesters of pregnancy
- Symptomatic hypotension may occur in intravascularly volume-depleted patients
- Significantly increased plasma concentration in cirrhotic patients
- Changes in renal function reported in renal impaired patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Lower starting dose recommended for patients with moderate to severe renal impairment
- Lower dose should be considered for patients with hepatic impairment
- Contraindications in pregnancy and hypersensitivity to losartan
- Elevated blood urea and serum creatinine in patients with bilateral renal artery stenosis or solitary kidney
- Not to be used with potassium-sparing diuretics
