Indications
- Hypertension
- Renal Protection in Type-2 Diabetic Patients with Proteinuria
Pharmacology
- First non-peptide orally active angiotensin II receptor blocker
- Binds to AT1 receptor in various tissues reducing vasoconstriction
Dosage & Administration
- Usual starting and maintenance dose of 50 mg once daily
- Total daily dose ranges from 25 mg to 100 mg
Interactions
- Reduction of active metabolite levels with Rifampicin and fluconazole
- Potentiation of antihypertensive effects with hydrochlorothiazide
Contraindications
- Contraindicated in pregnant women
- Not to be administered with Aliskiren in patients with diabetes
Side Effects
- Mild and transient effects such as dizziness and diarrhea
- Other effects include fatigue and abdominal pain
Pregnancy & Lactation
- Pregnancy Category D
- Risk increases if administered in second or third trimesters
Precautions & Warnings
- Reduces fetal renal function during pregnancy
- Symptomatic hypotension in intravascularly volume-depleted patients
Use in Special Populations
- No initial dosage adjustment in patients with mild renal impairment
- Use a lower starting dose in patients with moderate to severe renal impairment
