Indications

  • Treatment of hypertension
  • Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Angiotensin II vasoconstrictor
  • Aldosterone secretion stimulator
  • Thiazide diuretic effect
  • Renal tubular electrolyte reabsorption modifier

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dosage of 100/25 once daily
  • Antihypertensive effect attained within three weeks
  • No initial dosage adjustment needed for elderly patients

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug interactions found with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine and Phenobarbital
  • Potential interaction with potassium-sparing diuretics, potassium supplements, or salt substitutes
  • Interaction with alcohol, barbiturates, or narcotics causing potentiation of orthostatic hypotension
  • Interaction with antidiabetic drugs requiring dosage adjustment
  • Additive effect or potentiation with other antihypertensive drugs
  • Impaired absorption in the presence of anionic exchange resins

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria
  • Hypersensitivity to other sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reported angioedema, vertigo, gastro-intestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, sleep disorders, malaise, urticaria, pruritus, rash, hepatitis, atrial fibrillation, cerebrovascular accident, syncope, paraesthesia, pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, anaemia, thrombocytopenia, hyponatraemia, arthralgia, myalgia, renal impairment, rhabdomyolysis, tinnitus, photosensitivity, and vasculitis

Pregnancy & Lactation

  • Avoid in pregnancy unless essential
  • May adversely affect fetal and neonatal blood pressure control and renal function
  • Not recommended in breastfeeding

Precautions & Warnings

  • Angiooedema hypersensitivity
  • Periodic determination of serum electrolytes
  • Rare development of hypokalemia
  • Impaired renal function
  • Symptomatic hypotension
