Description

  • Losarcar Plus is indicated for the treatment of hypertension and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy. It is a combined antihypertensive preparation containing Losartan and Hydrochlorothiazide. Losartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II, while Hydrochlorothiazide is a thiazide diuretic.

Dosage Instructions

  • The usual starting dose is one tablet (50/12.5) once daily for hypertension. Dosage may be increased according to patient response. The antihypertensive effect is usually attained within three weeks after initiation of therapy.

Administration

  • May be administered with or without food

Drug-Drug Interactions

  • Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium may lead to an increase in serum potassium levels. The antihypertensive effect may be blunted by the non-steroidal anti-inflammatory drug Indomethacin.

Contraindications

  • Hypersensitivity to any component of the product. Anuria. Hypersensitivity to other sulfonamide-derived drugs.

Side Effects

  • Symptomatic hypotension, dizziness, hyperkalemia, angioedema, vertigo, gastrointestinal disturbances, angina, palpitation, oedema, dyspnea, headache, sleep disorders, malaise, urticaria, pruritus, rash, hepatitis, atrial fibrillation, cerebrovascular accident, syncope, paraesthesia, pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, anemia, thrombocytopenia, hyponatremia, arthralgia, myalgia, renal impairment, rhabdomyolysis, tinnitus, photosensitivity, vasculitis.

Pregnancy & Lactation

  • Should be avoided in pregnancy unless essential. Not recommended during breastfeeding.

Precautions & Warnings

  • Periodic determination of serum electrolytes, attention to the possibility of hypokalemia, impaired renal function, and symptomatic hypotension.

Special Populations

  • Use in Patients with Renal Impairment - Follow usual therapy regimens if patient's creatinine clearance is >30 ml/min. Loop diuretics are preferred in more severe renal impairment. Use in Patients with Hepatic Impairment - Not recommended for titration in patients with hepatic impairment. Use in pediatric patients - Safety and effectiveness not established.

Overdose Effects

  • Manifestations may include hypotension, tachycardia, and electrolyte depletion. Supportive treatment should be instituted if symptomatic hypotension occurs. Neither losartan nor its metabolite can be removed by hemodialysis.
