Indications
- Treatment of hypertension
- Reduction of risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Blockage of angiotensin II binding to the AT1 receptor
- Inhibition of aldosterone secretion
- Increase in plasma renin activity
- Increase in aldosterone secretion
- Reduction in plasma volume
- Inhibition of ACE (kinase II)
- Diuretic action on renal tubular mechanisms of electrolyte reabsorption
Dosage
- Starting dose of 50/12.5 as one tablet once daily
- Maximum dose of 100/25 once daily
- Antihypertensive effect attained within three weeks after initiation of therapy
Administration
- May be administered with other antihypertensive agents
- Can be administered with or without food
Interaction
- No significant drug-drug pharmacokinetic interactions found with various drugs
- Potential interaction with potassium-sparing diuretics, potassium supplements, salt substitutes containing potassium, and non-steroidal anti-inflammatory drugs
- Potentiation of orthostatic hypotension when administered concurrently with alcohol, barbiturates or narcotics
- Possible dosage adjustment of antidiabetic drugs (oral agents and insulin) required
- Additive effect or potentiation when used with other antihypertensive drugs

- Hypersensitivity to any component of the product
- Anuria
- Hypersensitivity to other sulfonamide-derived drugs

Precautions & Warnings
- Angioedema hypersensitivity
- Periodic determination of serum electrolytes
- Risk of hypokalemia
- Impaired renal function

Use in Special Populations
- Usual therapy regimens if creatinine clearance is >30 ml/min
- Loop diuretics preferred in patients with more severe renal impairment
- Not recommended in patients with hepatic impairment
- Safety and effectiveness not established in pediatric patients
Overdose Effects
- Manifestation of hypotension and tachycardia
- Possible bradycardia from parasympathetic stimulation
- Supportive treatment required in case of symptomatic hypotension
