Indications

  • Treatment of hypertension
  • Reduction of stroke risk in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Angiotensin II vasoconstrictor
  • Aldosterone secretion stimulator
  • Vasoconstrictor and aldosterone-secreting effects blocker
  • Renal tubular electrolyte reabsorption affecting diuretic
  • Plasma volume reducing diuretic
  • Potassium loss increasing diuretic

Dosage

  • Usual starting dose of 50/12.5 tablet once daily
  • Maximum dose of 100/25 once daily
  • Antihypertensive effect attainment within three weeks
  • No initial dosage adjustment necessary for elderly patients

Administration

May be administered with or without food

Interaction

  • No significant drug-drug interactions found
  • Potential interaction with potassium-sparing diuretics
  • Interaction with alcohol, barbiturates, or narcotics
  • Possible dosage adjustment required for antidiabetic drugs
  • Additive effect or potentiation with other antihypertensive drugs
  • Impaired absorption in the presence of anionic exchange resins

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria
  • Hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reported cases of angioedema
  • Less commonly gastro-intestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, sleep disorders, malaise, urticaria, pruritus, rash
  • Rare cases of hepatitis, atrial fibrillation, syncope, paraesthesia
  • Reported cases of pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, anaemia, thrombocytopenia, hyponatraemia, arthralgia, myalgia, renal impairment, rhabdomyolysis, tinnitus, photosensitivity, vasculitis including Henoch-Schonlein purpura

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • May adversely affect fetal and neonatal blood pressure control and renal function
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Risk of hypokalemia
  • Impaired renal function
  • Symptomatic hypotension

Use in Special Populations

  • Use in Patients with Renal Impairment
  • Use in Patients with Hepatic Impairment
  • Safety and effectiveness in pediatric patients not established

  • Manifestations of hypotension and tachycardia
