Medicine Details

Indications

  • Treatment of hypertension
  • Risk reduction of stroke in hypertensive patients with left ventricular hypertrophy

Pharmacology

  • Losartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily for hypertension
  • Initial treatment for severe hypertension is one tablet of 50/12.5 once daily
  • Antihypertensive effect is attained within three weeks after initiation of therapy

Administration

May be administered with other antihypertensive agents. Can be taken with or without food.

Interaction

  • No significant drug-drug interactions with Losartan
  • Interaction with thiazide diuretics may potentiate orthostatic hypotension
  • Dosage adjustment of antidiabetic drugs may be required when administered concurrently with Hydrochlorothiazide

Contraindications

  • Contraindicated in patients hypersensitive to any component
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness may occur
  • Hyperkalaemia and angioedema may occur
  • Other side effects include vertigo, gastrointestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, etc.

Pregnancy & Lactation

  • Avoid in pregnancy unless essential
  • Not recommended in breastfeeding
  • May adversely affect fetal and neonatal blood pressure control and renal function

Precautions & Warnings

  • Periodic determination of serum electrolytes should be performed
  • Hypokalemia may rarely develop, especially with brisk diuresis
  • Impaired renal function and symptomatic hypotension should be monitored

Use in Special Populations

  • Use with caution in patients with renal impairment
  • Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Manifestations of overdosage include hypotension, tachycardia, and electrolyte depletion
  • Supportive treatment should be instituted for symptomatic hypotension
