Medicine Details

Indications

  • Treatment of hypertension
  • Risk reduction for stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Losartan and its active metabolite block vasoconstrictor and aldosterone-secreting effects
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption
  • In vitro binding studies indicate losartan is a reversible, competitive inhibitor of the AT1 receptor

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily
  • Antihypertensive effect is attained within three weeks after initiation of therapy

Administration

  • May be administered with or without food

Interaction

  • Concomitant use of potassium-sparing diuretics or salt substitutes containing potassium may lead to increase in serum potassium
  • Alcohol, barbiturates, or narcotics may potentiate orthostatic hypotension when administered concurrently with thiazide diuretics
  • Dosage adjustment of antidiabetic drugs may be required when administered with hydrochlorothiazide

Contraindications

  • Hypersensitivity to any component of the product
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Hyperkalaemia occurs occasionally
  • Angioedema has been reported with some angiotensin-II receptor antagonists

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding, alternative treatment options are available

Precautions & Warnings

  • Periodic determination of serum electrolytes to detect possible imbalance
  • Hypokalemia may rarely develop
  • Impaired renal function and symptomatic hypotension should be monitored

Use in Special Populations

  • Use in Patients with Renal Impairment: Usual therapy regimens may be followed as long as creatinine clearance is >30 ml/min
  • Use in Patients with Hepatic Impairment: Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness in pediatric patients have not been established

Overdose Effects

  • Manifestations may include hypotension, tachycardia, and parasympathetic stimulation
  • Supportive treatment should be instituted if symptomatic hypotension occurs
